1 · About your practice

Tick every type that applies — a practice can be more than one (e.g. an optical shop with an optometrist on site).

Shown under your name in search results. 90 characters.

Square works best. Without one we show your initials on a coloured tile — never a blank space.

Wide shot across the top of your profile — the shop front or the dispensing floor. Around 3:1.

Up to 8. JPEG, PNG or WebP, 5 MB each — anything larger is skipped rather than failing your submission. You can caption them later from your dashboard.

One profile, every office. Patients searching any of your ZIP codes find the same listing, so you do not have to keep several near-identical profiles in step. Leave a field blank and that location falls back to your main phone number or booking link.

Each office keeps its own opening hours, set from your dashboard once the listing is live. An office that keeps the same hours as the practice needs nothing — it shows the practice's, marked same as the practice. A branch in another timezone can set that too.

Paste the full address or just the handle — we work out the rest. Links you leave blank simply do not appear on your profile; there are no empty placeholder icons.

2 · What patients can filter you by

The most-used filter on the site. Tick every plan you are in-network for.

Only tick brands you keep on the floor — patients filter by these and will call to ask.

Shown on your profile with credentials. Patients looking for a licensed eye doctor are told which of your people are one — an optician listed as an OD is the kind of error that costs a practice its listing, so the last field matters.

3 · How you operate

Type the name exactly as you know it. If you run on paper, spreadsheets or a booking tool only, write that — “none” is a valid answer. We use this to know which systems to build hours, availability and appointment-request sync for, so your listing can stay accurate without manual updates.

If you have an online booking page, patients get a Request Appointment button on your profile. Leave it blank and your phone number stays the main call to action — we will not send people to a page you do not have.

Plain language beats marketing copy here — this is the text patients read before calling.

Opening hours are set per office from your dashboard once the listing is approved, so you can adjust them without resubmitting.

4 · Who is submitting this

We send a verification link here — a business domain speeds up approval.

Listings are reviewed within two business days. We verify the address and phone number, and spot-check insurance participation. Nothing goes live until you confirm your email.
