Remove Putnam Parkway Eyecare from EyecareSearch

This takes the profile off the site. Patients searching your city, insurance or brands will no longer find it.

We will email a confirmation link to the address on this listing. Not to an address typed here — that is the only way we can tell an owner from anyone else who happens to be looking at the page.

It only helps us fix whatever prompted this. It does not change what happens next.

Cancel, keep the listing
Would correcting it do instead? Wrong hours, plans you no longer take or a stale phone number can all be fixed without losing the profile — sign in to your dashboard and edit them yourself.
