I would strongly caution others before choosing this facility for a loved one. Unfortunately, my family has had a very disappointing and concerning experience. There have been repeated issues with communication, including difficulty reaching staff by phone. More importantly, I have serious concerns about the level of care my aunt has received. Her needs have not always been addressed in a timely manner, and we have concerns about whether her medications are being administered properly. On one occasion, my aunt asked for something to drink and no one brought it to her. She ultimately had to leave her room and go to the nurses' station herself to ask for something to drink. This is extremely concerning when someone is supposed to be receiving care and assistance in a rehabilitation facility. My aunt entered this facility doing remarkably well, but over the past several weeks, we have seen a significant decline in her condition and overall well-being. She is no longer answering her phone, spends most of her time lying in bed, and seems withdrawn and unlike herself. It is heartbreaking for our family to see this change. We entrusted this facility with the care of someone we love and expected her basic needs, medications, and overall well-being to be treated with attention and compassion. Instead, we have been left deeply disappointed and concerned. I sincerely hope the facility takes these concerns seriously and evaluates the care being provided to its residents. Families should be able to feel confident that their loved ones are safe, cared for, and treated with dignity and respect.

Provo rehab is extremely easy to work with. I’ve worked with their discharge planner Mary Lou several times and she has been extremely professional and super quick to respond. Highly recommend their amazing facility. Not enough good things to say

Super nice staff. The showed me where the room was at and it was super clean. And the physical therapists and other therapists their are super awesome, they know what the are doing and make sure to keep them active and also safe on equipment

The chair in my fathers room was broken his bed was broken, they took forever to answer call light, he was newly aputated patient who did not receive physical therapy. My mom would have to go down to the laundry to look for his clothes they never returned them, the food was extremely subpar. Very understaffed we couldn’t get him out of there fast enough.

Jayden is amazing! As soon as I got on the Third floor to see my friend he guided me where I needed to go. He said it was only his second day working here! Additionally, when maintenance was needed for my friend‘s bed he took care of it right away and told me not to worry about it to just visit with my friend. I am so impressed with his ability to not only work with others that care for patients here on a very genuine and sincere level. He made my visit today so much easier and peaceful without worry.
