I had two different eye surgeries at Banner and I could not be happier with the results of the surgeries. Everyone at the facility was very informative and very professional. I would definitely go back if needed and would recommend this facility and the doctors to everyone.

Update. They put the wrong IOL in, falsified records and Banner Risk management doesn't care. There is no way everything in the chart was done considering I had one appointment before my first cataract surgery. I found out from reputable doctors that's not possible. Time to get an attorney. Nope. Don't do it. Go anywhere but here. Doctors are bailing like seniors after finals. I had surgery with one doctor. Two weeks that doctor left and was transferred to another doctor who was rude and condescending. He was supposed to refract me but gave me my prescription on a post it note. And found out today that he's leaving too. No loss there. I had asked the dept manager to call weeks ago. Never heard from her. I'm having really a really hard time seeing. Apparently that's not important. Just this year, I have lost 6 doctors at Banner. The only department I've had luck with is the infusion clinic. Even Rheumatology itself is so so since doctors are always leaving.
Worst clinic ever. Be careful of a SCAM. My kids went there for a regular check up, but they billed a high amount and when I requested to talk to ask why, they told me to talk to my insurance. My insurance called and they couldn’t provide any information for the insurance. Be careful.
Dr. Kozak is great. Quick appointment with photos and eye pressure testing, including standard vision exam. The waiting room is large and spacious, helps to ensure social distancing is still possible even when many patients are waiting to be seen. I dropped off a DOT form to be completed by the doctor on Friday. We'll see how long the turn around time is on that. Hopefully as expeditious as the rest of the scheduled appointment was. I've gone reason to complain thus far. Recommend seeing Dr. Kozak to anybody reading these revjews.
Thank you to Dr. Jones on the 3rd floor! Last time my son was here was 2yrs ago. & we didn't want to come back because we had a really bad experience with a different doctor. My son is a hospital baby & has been through a lot. He is considered special needs & sometimes I feel like some people & doctors just don't understand. Dr. Jones today was AMAZING, she took her time with our son, understood he needs & understood him! We got the answers we were looking for as much as we could get & it was just over all amazing for us. Thank you again for making this appointment go smoothly as possible.
